3. Audition Repertoire Change Form
   • It is important to note that any changes to audition repertoire must still meet the Audition Requirements. Incorrect or insufficient repertoire may affect consideration for admission.
   • Piano & Collaborative Piano applicants must have all audition repertoire changes approved by the Department in advance of submitting this form. Please email repertoire@juilliard.edu for approval.
   • You must bring at least 5 photocopies of the completed form with you to your audition. You may wish to refer to the number of faculty for your instrument in case more copies are needed.
4. Teacher Preference Form (Classical Music)
   • If you wish to change or update your teacher preferences, please complete the above form and mail or fax (212/769-6420) it to the Office of Admissions.
